depression questionnaire

I was at Dr's a couple of weeks ago I've to get bloods to check my hormone levels possible early menopause due to hysterectomy at 30 but she handed me a questionnaire as I keep breaking down and losing it with DH and the kids I was told to answer it honestly but if i do it makes me look such a bad person I've probably been depressed for a good few years but I can't hide it any more just feel so down. Don't know what to do due to go back Friday.

Just answer it honestly and let the doc recommend how you proceed from there.
It won't make you look like a bad person. Remember the doc has seen it all before

There's no need to hide depression. It's not something to be ashamed of. Answering the questionnaire will not make you look like a bad person.

Why do you feel like you have to hide this and deal with it alone? Your GP can help.

After suffering from depression all my life and finally finding the right medication for me I can honestly say I feel like a new person! I had no idea I could feel so normal.

filled it in honestly but she didn't believe me however she thinks it menopause as well so to start oestrogen first don't want AD's been there before made me worse.
