DD with high temp-advice please

My DD 22 months woke up at half 4 this morning and vomited. She had a temp of 39 and was very upset and crying. She was asking for water so had some water and calpol/neurofen and about 40 mins later vomited the water up. The temp hasn't went down despite another dose of calpol this morning. She is very sleepy and unhappy. And keeps tapping her head so I think she has a headache. Cold hands and feet. No other signs of any illness- no cold, cough or rash, or sickness bug (she is only ever sick when her temp spikes) She is drinking water, WWYD?

Same as I've been dong since the early hours of Christmas Eve. Keep offering water sponge down if hot and keep up the alternate doses of cal poly and nurofen. Ds2 didn't keep anything down from 1am Christmas Eve until a bowl of cereal yesterday late afternoon.
He's exhausted now but much brighter, temp under control. Keep offering fluids if she keeps being sick some flat full fat lemonade or coke can get some calories into her. It may come back up but some of it will hopefully stay down after the first few hours he was only sick when his temp spiked as well I ended up opening a window and cooling down the whole room because he wanted cuddles but I was making him warmer.

Has she had a wee today? Is she drinking fluids ok?
Have you stripped her off? Nappy only?
Is she playing?

Keep her hair wet, use damp cloths to cool her, one around her neck. Let her suck ice lollies. Do you have any yogurt sticks, like frubes? Mine would eat those to help with temperature. Keep a close eye on her.

Thank mumsnetters for quick replies.
She's been stripped down for a while. She has had a couple dark yellow wee's. She's been potty training and is refusing a nappy so has went on the toilet. She is very thirsty and keeps having sips of water. She ate a yoghurt fine earlier about 10am but nothing since. She just feels very hot still. Not playing with her new toys, just wanting to cuddle. No sickness since she was sick initially early doors. Viral? I'm hoping it's just a 24 hour fever. She had a medium temp for a couple days last week but wasn't unwell in herself. She has a red bruise looking mark under her armpit that appeared last week and hasn't faded at all, so I think I'll get her an appointment anyway tomorrow. It looks kind of like a red birth mark that appeared but it's never went away for about two weeks now so it's probably not linked.
I'll see if I have ice lollies. If not I'll get DH to pop out.

I was on phone to 111 about Ds with high temp and they said definitely do not sponge down or fan as temperature drops too quickly which creates shivers and causes it to go higher. Also told not to bath as gets cold too quickly when they get out. Turns out ds has chicken pox. Hope your DD is feeling better soon

Sponging down isn't recommended as it can trick the body into thinking it's cooling down and then the brain makes the temperature higher. If the temperature isn't coming down with calpol and nurofen I would phone 111 for advice, especially with a headache and cold hands and feet. Don't worry about food, keep up the fluids.

What's her temperature now? To be honest headache cold hands and feet plus high (or low) temperature I'd be considering 111. Probably just a virus maybe sinus or earache but worth being cautious.

Hi, sorry we were at my mums for a bit where she seemed a bit better. Her temp has been holding at 38.6, but her hands and feet are still freezing. She can have another dose of calpol and neurofen but I was trying to hold off until bed. She didn't want any dinner, and hasn't drank very much this avo. She's tired but alert IYKWIM.

Best giving the calpol and nurofen separately in case of vomiting again.

With the nurofen being 8 hourly, I generally time it to cover the night, so last thing before bed time.

Really push the fluids, every 10 minutes get her to drink, she needs more with a temperature. Jelly, yogurts, ice lollies are all good.
Don't wrap her up with blankets tonight, just a nappy.

Ah that's good, still give the calpol or nurofen whichever is due, please don't think that it will mask her temperature....many people do this, believe it or not!!

Ok seen the GP and she's went straight to bed. They think just a nasty virus, but she is showing signs of dehydration, so if she doesn't drink any more, and isn't producing urine she might need some IV fluids tomorrow. Hard now that she is sleeping though to get fluids in her . Fingers crossed for a good night. Thank you everyone

Cold hands and feet and high temp can be a sign of sepsis. Please google the symptoms and take her to A and E if necessary. Hopefully it's just a virus however. Hope she's feeling better soon

As a pp said, cold hands and feet are one of the sepsis 6 pathway.

Eek I hadn't thought about sepsis, and I've had it twice! I think ill sleep in the rocking chair next to her tonight so I can be close by.

Cold hands and feet with high temperature is meningitis! You don't always get a rash.... In fact a rash is a v late sign. If she still has a temperature and cold hands and feet I would take to hospital. At least get a second opinion.

Cold hands and feet with a temp are a warning sign of sepsis, and I think of meningitis. The last time I took one of mine to the GPs with a temp, they felt his hand for this. I am risk averse, but I would be off to A and E now.

Me too op - she has all the classic signs of meningitis. Sadly you can't rely on an out if hours dr to catch this - get a second opinion.

Trust your gut instinct, look out for change in temperament or responsiveness. Other possibility is a UTI, see if you can get a sample of urine asap to take with you whenever you see doctors. Good luck

starsandcars your posts are very alarmist, correct me if I'm wrong but I don't think any of us are qualified to diagnose someone over the internet. OP if you're still worried you should seek a second opinion in person no matter the time of day or night.

Cold extremities are a red flag .

Please do not use damp flannels to bring down temp ... Some of the advice on here is very out of date. ( I'm a senior nurse )

In such a young child with a temp of 39/40 , lethargy , cold extremities I would be seeking out of hours advice .
