Fluconazole dosage advice please

I've had my second bout of thrush now for over a month. Third GP at my practice prescribed Fluconazole but only the 150mg loading dose so it hasn't improved. It is at best uncomfortable to feed and often agony for hours after. I've had my latch checked at our local Breaststart drop-in and it's fine.
So....my sis had bought Flucon in Cambodia and brought it back for me (genuine thing don't worry!) but it is in 200mg tablets. Easy to break in half but not so easy in quarters. So really my question is could I have a 200mg loading dose and then just one 100mg dose per day or will I have to chop them into 50mg pieces?
Thanks in advance.

I think the reason he only gave you the loading dose is that fluconazole is not normally recommended in pregnancy and the manufacturer says it should be avoided in BF.
My advice would be to go back again. I sbaby being treated aswell with drops?

There is no point in going back to GP as none of them would read the BFN printout and one said that he didn't believe that thrush could occur in breats. The 3rd one was a woman (she is usually the best one) and she said that sometimes it cleared up with the one dose but I reckon that's a bit daft as she was willing to take liability for the drug but just not for the full course....
L&Smum, yes we have gel for nips and babe and also Nystan cream which seems better for my nips.
Good luck with your GP kayzed - if not ring round your friends and see if any are going overseas!

I'd be interested to know how you get on with this. I had almost continous thrush for the first 6m when feeding DS1. DS2 is now 2 weeks old, and we both have it I had a/b's in labour, and he's on prophylatic a/b's for kidney problems, so it was almost inevitable.

What treatments do you feel work best? Might push for the fluconazole at an earlier stage this time.

apparently, some women swear by washing the nipples in vinegar after feeding, washing then allowing to air dry.
also have you tried dosing yourself up with bio friendly yogurt which can help keep candida under control.

laundrylover · 15/07/2006 22:04

Thanks for the link Kayzed but I have tons of the 200mg tablets already (only $2 the lot from Cambodia!!)so am chopping away!
Hats off to you Laura for doing 6 months but keep pushing this time as having experienced pain free feeding last time it's sooooo much better!!
SC - am eating yoghurt but not tried it on my nips yet. However I do think that the full course of drugs is going to be needed as I've had it for a while now and reckon it's in my milk ducts.
Will let you all know how I go on over the next week or so....

treatment depends on where the thrush is located in the breast, has the gp tried you with topical nystatin?, which is what im guessing they have given you for your baby.
flucon, is not strictly recommended in pregnancy or breast feeding, but gp's will prescibe it, the usual dose being 100mg a day. it is secreted into breast milk.

I didn't have the fluconazole last time. We had the nystatin drops for DS's mouth, timodene for his bum, canesten cream for my nipples, and I used the canesten duo set down below.

I think I coped with it for six months as I didn't realise that it shouldn't be feeling as painful as it was. We managed to clear the symptoms of thrush from DS1 (nappy rash and in his mouth) quite easily, but I suffered the whole time.

In the end it was a course of four pessaries - one per week for a month, that cleared it down below for me, and then the thrust in my ducts seemed to go as we weren't passing it round!

If you've got it down below as well, try the cream 'pessary'. My MW suggested it, as 2 weeks after the birth, the pessary didn't appeal to me either. I've got stitches inside and out, and didn't have any irritation at all from the cream.

My MW didn't mention any reason not to use the cream/tablet pessaries because of an open cervix. Hope it's all OK!

I'd also advise hot washing all towels/knickers & bras. 60 degrees at least - or sterilise with some Miltons.

I've only had it a few days this time (DS2), but suffered for a long time with DS1. After DS1 it took 6m to fully get rid of it.

laundrylover · 16/07/2006 22:14

Gosh girls you two are having a worse time than me!
I have Nystan cream for my nips but instead of the nystatin drops I use Daktarin oral gel for DD2's mouth (even tho no symptoms) as some research I read reckoned the active ingredient was better.
I know what you mean about wearing clothes - for me the worst is a dressing gown - and seem to get perma nip on!!! Not an attractive look for me.
Anyway my after bfing pain is MUCH less and didn't keep me awake last night so hope this is the flucon working....
Do you know that you can take Ibruporen too which does help for instance if like me you are more milky at night.
Good luck the pair of you.

DD2 is 15 weeks so a good bit older than yours. I believe that there is some promotion of Flucon around as a newborn drug?? Dosage thru milk is only 0.06mg and recommended for babes is 0.6 so it won't even reach a level to treat the baby. This is all from memory tho and I know you've looked all this up. I can understand you wanting to wait - are you taking Ibrufrofen for the pain at all?
It makes me really angry that GPs won't help with this - it should all be clear cut and above board not making us feel like druggies!

I think I'm getting better so am keeping going with the flucon course. def not making me yelp when she latching on and I think the pains after feeding are fading.
Good luck with the next two weeks - have you bought some Flucon to be ready?
Is it any wonder so many people give up on bf??
