Diege usually the CMW also work shifts in the hospital. If you've booked for a HB (in my area) then 2 mw's from the hospital/on-call team will attend.
The first mw is usually released from the hospital if it's after 6pm, the second will arrive if the first calls to say it's an active labour. The second mw is the on-call mw.
If you labour during the day the CMW will attend you first and ring labour ward to let them know it's active labour. They then will usually send for on-call if they are pushed, or sometimes another member of the CMW team turns up.
HB will take precedent, it's your right to give birth at home. They can spin you whatever line they like but the mw have to come.
There has been legal precedent set when a Trust a little while ago refused to have mw's attend a planned hb due to staff shortages. Ruling stated the Trust would be negligent and ordered them to fund an independent mw for the lady concerned.
Re: worries of Ds getting stuck, they won't have ventouse but might help him out 'old style', rocking his head or releasing his shoulders. This has happened to me twice (shoulder dystocia with Ds1 and ill-presented head with Dd2) It was dealt with very efficiently and effectively by the mw's.
If for whatever reason they suspect something is amiss they ring 999 and blue-light you to hospital, but it is quite rare for this to happen. The hospital will be ready for you and aware of the situation when you arrive.
No.4 will shoot out! I think that's why Dd2 had a bit of a problem with her head tbh. She descended so bloody fast (approx 3 minutes) that she didn't get enough time to turn so her shoulders were in line.
