I wonder if the 'surname as middle name' thing is because the parents may not have been married legally.

I think it's just fairly traditional in some areas. Certainly the families I know who did (and continue to do) this were mostly very traditional religious families, so were definitely legally married.

The mother's maiden name as the first son's surname is quite traditional where I live too. Lots of men through the generations called things like Barclay, Campbell, Thompson etc.
