Letter to change maternity start date

Hi,

Again I am turning to all your knowledge with my work problems...

I know want to write a letter changing my maternity start date and including my annual paid holidays (I have to shorten to 11 months in order to obtain the 28 days paid holiday).

Are there any particular points that MUST be stated in this letter??

Assuming you are giving the required 8 weeks' notice you just need to write and say that you are changing your date of return from maternity leave from x date to y date. If you then want to take some annual leave you should say something like 'I understand I have x number of days annual leave outstanding and I would like to take x number of these between x and y dates, meaning I will return to the office on x date. Please confirm that my holiday request is acceptable.

Something like that. You need to know that your employer doesn't have to necessarily agree your holiday request, you are entitled to accrue holiday during your maternity leave, and taking it in a lump at the end is sensible, but although you can dictate the dates of your maternity leave, the holiday is a request you make so you should phrase it accordingly.
