URGENT - what do I need to include on an invoice?

I'm sending out my very first self-employed invoice but have just remembered I lent my idiots guide to accounts to a friend.

What do I need to include on my invoice? So far I've got:

My name & address
Client's name and address
Invoice: Project name
What for and how much
Date

Is there anything else that should be on there?

Invoice number
Invoice date

Vat reg. number if you have one.

Add VAT clearly if you charge it.

Company reg. number if you have one.

Payment methods you can accept and bank details for them to pay by bacs if they wish to and you can accept it.

Don't forget to include the date by which payment is due.

I'm a bookkeeper who specialises in startups if I can help at all. Very reasonable rates.
