Is there a standard format for a letter of resignation?

Does this sound alright or is there some protocol I should be following?

Dear xxxx

You can put a reason in if you like but it's not mandatory, nothing is! I would usually include sometihng saying "as my notice period is x my last day will be Y"

And if you like them and are leaving to go somewhere else blah blah then you could put something about having enjoyed working there etc. Up to you.

Standard I would use would say

Dear Tosser McToss

This is to inform you of my resignation from my position of cxxxxx. As my notice perid is 1 month my last day will be ccccccc

Yours etc

Thank you for that. I am trying to refrain from the tosser mctoss until my exit interview!! Plus I like my manager - it's the person above her I don't like very much!!

I have already told her on the phone and she was so nice about my work there and how much I would be missed that she actually brought a tear to my eye.

My last day thing may be tricky as I was due back from mat leave on July 15th so I don't know if that affects it? I will also have god knows how many accrued holidays.
