Maternity Pay SMP and MA

Hope someone may be able to advise. I’ve been employed by NHS for over 10 years. I fell pregnant in the first week of starting with a new NHS trust and have only recently learnt that due to not working with the new trust for 26 weeks prior to 15th week before baby was due I’m not entitled to statutory maternity pay (SMP). I have been entitled to occupational maternity pay (OMP) for 26 weeks of maternity leave due to my continuous employment with NHS. My baby is 6 months now, is it too late to apply for Maternity Allowance (MA)?

I’ve learnt I need a SMP1 form to apply for Maternity Allowance, which work haven't provide me with.
