How do references via HR work?

NHS to NHS job change - the reference from my current employer is to be sent to my current NHS employer's HR team - is it the HR team who process and return the reference? Or will HR receive it, forward it to my line manager to complete, for my line manager to send it back to HR and HR send it back to my new employer's HR? Things are really busy at work at the moment and I just want to know if I should give my manager the heads up that they'll need to take some time to complete this for me. If the reference is dealt with totally by HR then I won't, just a bit confused as to how it works in the NHS! Thank you x

Not sure about NHS but local authority is pretty much confirming role held and dates employed by them
