need 3 separate weeks off next year but applying for new jobs - Do I put it on app form

I have seen a really interesting jobs i want to apply for but I will need 3 different weeks off 1 in may (hol booked) 1 in June (to cover childcare) and 1 in Sept (hol booked) None are in school hols. I think I should put it on app form or is it too forward. I feel if they know up front and it is too awkward for them they wont even interview me. what is point of wasting their time. Any opinions????

I would only ask at the interview stage whether or not they could honour holidays etc. There's prob no need to mention it on application forms.
