If you feel able to, I'd ask her how she'd like the operational logistics to be handled as this seems the most sensitive way.

She wants to try and join a team meeting to say goodbye.
I would let her do this as it is her wish. You will need to prepare the team so they are aware. You mention HR is outsourced, do employees have access to an EAP that can offer them counselling/someone to talk to confidentially? If EAP is not an option then possibly have a second meeting without your boss to let anyone who wants to have the opportunity to debrief. Everyone reacts differently so no requiring people to share feelings etc. But allow those who want to to talk. www.acas.org.uk/time-off-for-bereavement/if-someone-at-work-dies

What do I do about getting her equipment back?
Make a list of equipment so you don’t forget anything. As pp suggested ask her what she would prefer. Offer to have it collected at a time that suits her or her family.
Do I leave her systems access live till she's gone?
Yes.
