I received an application form for a job I saw advertised and the company are asking for me to disclose any health conditions. Can they do this?

Not really, but they do. You have a choice not to declare anything and wait to see if you get the job. Then carefully disclose, wait and see if they sack you. Go to tribunal for disability discrimination.

Join a Union now so that you have their support and negotiating power from the very beginning. I've been there, done that. Lost a job before I started when I asked about disabled parking spaces.

I don't mind disclosing if I was too get the job but on an application form it worries me who in the workplace will see it. Its an NHS job and I know a few people that work in the department and I wouldn't want them to see my application form with my health conditions on.

I don't know how it works in the NHS, but often these questions (along with anything on disability/ race/ sexuality ect) are only seen by HR and removed before the forms get to decision makers.

It's so they can monitor whether the process is fair to various groups and check whether you need any adjustments for the interview.
