Do NHS reference give details of disciplinary (Capability Policy)

I'm facing possible HR action in relation to some performance issues at work. My line manager has followed the trust's capability process. Informal action plan didn't bring about the changes expected of me, so I think she's taking it up a notch with HR. Meanwhile, I'm applying for other jobs (have never been suited to this one, just landed in it by chance when I need something local to fit around my care committments),
Do details of capability related disciplinary proceedings appear on NHS references?
Can anyone from NHS HR confirm this please?

If informal stage it doesn’t go on references, if formal stage and you are still going through it when you are offered a new job it does, unless you go through the capability policy and manage to get yourself off it. It’s horrible I know, and really takes it’s toll emotionally. Capability isn’t disciplinary though, although the outcomes can be similar.
