Informal action plan (NHS), anyone HR here?

Last year I was put on an informal action plan at work due to some mistakes I was making. I later disclosed to my boss that 8 was having issues at home (domestic abuse) that was impacting my work. When I disclosed this my boss was sympathetic and changed it to a support plan instead. With counselling and GP help I managed to get better but suffered a relapse a couple of months ago.
The relapse happened after I'd moved to a new job within the same NHS Trust, so my file was sent to my new manager.
Meanwhile, I've been put on an informal action plan in the new role as have been struggling due to working remotely and postponed training etc.
Can anyone with knowledge of NHS HR please advise whether these 2 different informal plans will follow me if I move jobs again with the same trust? I'm currently applying for jobs closer to home but feel dubious about 2 plans from different managers showing on my record x

I'm not sure what you mean by relapse. Could you say a little more?

I’m NHS HR. Is the plan under the Capability Policy? If so, it wouldn’t automatically follow to a new job but if similar issues occurred it could be restarted.
