Job Application with a health condition - Help with disclosure

I have two long term health conditions (psoriatic arthritis and CFS).
I want to apply for a full time job but not sure how/when to disclose my illnesses.
The arthritis is a wax and wane condition which often causes no problems and the CFS has eased considerably over the last two months.

The application has a section in the person spec worded like this:
To be assessed as medically fit in accordance with Occupational Health
Department to perform the duties of this post

There is no other section on the form to disclose health issues.

Could anyone advise me on what I should do please?
I think I can do the job, but as with any wax and wane health issues, I have no idea how long away I am from a flare up or a change in my condition.
I want to be entirely honest but don't want to lose out because of it.

I wouldn't disclose, it's for OH to determine if you're fit enough if you're offered the job.

As late in the process as possible, I say. If you don't need any accommodation for the interview, say nothing about it until after you've started the job!

Because the reality is if you mentioned it early, a lot of employers will unlawfully discriminate against you, and unless they're careless enough to admit it in writing you have zero proof.

When answering the section in the person spec in my statement in the application I guess I could confirm that I would be happy to under take an OT assessment. Would that cover me do you think?
