Anybody knowledgeable on sick notes and phased return?

I've been off work sick for 3 months. Been feeling a lot better and about to start a phased return to work, with a really understanding and helpful employer but had a rough couple of days and started to doubt ability to go back next week. I need to speak to doctors tomorrow and request either a sick note or one that says "you may be able to return to work if..." - my question is if I request the latter and still feel this ill next week, does it still cover me in the same way? I'm on full pay for sick leave at the moment.

If your dr gives you a sick note (it has all options on it, it's just one note) that covers you from the day you are due back to work until whatever date the dr believes you need to get better, the note covers you the same way. That just covers the continued absence keeping it at one period; whether you're paid or not depends on your employers sick pay procedures.
