Return to work letter - help please!!

hi

I'm due to return to work on 6 August and I've been asked to put the details in writing. I was working full time but now due to childcare issues I can only work 2 days a week Mond & Tues. As far as I know this should be ok with the company I work for.

What should I put in the letter, I'm rubbish at drafting letters and everything I write seems to be too much to the point.

Can anyone help or have any ideas to point me in the right direction?

I turned the text into a letter, the key thing is to include the bit about making an application under your rights.
