Mental health and Health Questionnaire...

My cousin’s daughter (she’s 19) took an overdose 18 months ago. It was following a particularly bad time she was going through and she was eventually prescribed Sertraline. It was only after the attempted overdose to finish her life did she admit to feeling so out of control in her head. She immediately came off them having spent 7days in hospital. So,, fast forward she’s now looking for s job, found one, got accepted and they have sent her a health questionnaire asking some detailed questions regarding her health, or. Has she suffered with her mental health, etc.
Should she say that she has or will she be wise to keep quiet?
I just don’t know what to say to them. Any advice welcome, thanks

I would give full disclosure; were she not to and the truth to emerge she would face dismissal. Added to this the knowledge will allow her new place of work to support her if ever needed.
