Please help me write this important letter to my boss - I've got brain freeze!

I have to formally write to my boss (I'm a teacher, so our headmistress) to inform her of when I intend to start maternity leave but also to tell her of my intention not to return to work after my maternity leave finishes. I can't even begin to think where to start and I'm awful at writing formal letters, does anyone have an example or any suggestions please? Any help would be really appreciated, I truly have pg brain at the moment!

are yoiu sure you have to write a letter?
I'm a teacher and have had 3 mat leaves - not one letter, I just informed them of it all - ONly had to write a letter with intention to return dates

the LEA sends through paperwork and they have a tick sheet with options, had to send that back and sign it with my chosen option

Afraid the letter is a necessity - our LEA have just changed their policy . It's more the not returning part I'm worried about writing!

I would make a big deal about letting them know now so they can plan their staffing well in advance.

Oh I've let them know ages ago that I intend not to return (they've already employed my replacement!), it's the formal letter part I'm having trouble with - and I'm supposed to be a literacy teacher!!

As you are aware from our conversstions, I am expecting a baby on XX/XX/xx.

I am writing to confirm that I intend to start maternity leave from xx/xx/xx. Should there be any reason for a change to this date, I will, of course, notify you as soon as possible.

I would also like to formally confirm that I do not intend to return to work as XXX at the end of my maternity leave.

I'd like to take this opportunity to thank you and the other staff for your support and ???? over the last xxx years. I ahve enjoyed my time at ZZZZZ school immensely.
