Long Term Sickness Resign or Extend or Partial Return?

Lovely employer, enjoy my job, well paid professional job, very flexible on hours and unpaid leave just to give background.

Been there just under 2 years. Started becoming very ill a year ago initially agreed with work could wfh 4 days a week one day in office at OH suggestion. Doctor started doing lots of cancer tests, he said he didn't think I had cancer but sent me off for test after test. All negative. Personally I don't think its cancer put on double antibiotics and it helped but only 14 days and relapsed since. Doctor wanted me to carry on wfh 5 days but OH said too ill to work. Doctor agreed to sign off but won't give a diagnosis or any treatment so I have been made to have 2 months off (at full pay) but no improvement.

Doctor said to OH its infections plus anxiety though OH questioning this and suggesting neurology brain and spine MRI. Doctor says no. Tbh I don't think its that either but symptoms match Lyme and I had a strange bite before I got ill which matches one of Lyme rashes a year ago and symptoms match. Doctor suggested I get a private referral for testing private doctor said no I am imagining being ill and if I just go out I will feel better. So no help and feel awful, about 2 weeks out of 4 I just feel like mild flu, 2 weeks I tend to need constant antibiotics or paracetamol, sometimes I will just collapse on standing, only about once a month.

Can't see much hope of getting better so would you resign or extend sick leave or try partial return? We can manage on one income, more the thought of not being able to work that is depressing. Its also well paid and interesting and pays for our summer holidays. I have moved next to DHs work and got a school there very close by that's excellent for the children does mean its a long journey from my work. I would only get SSP now but that's not an issue. Contract just for another 6 months or so. Work wanted me back but now think I am not well enough, they advertised for temp before but didn't get anyone. Thanks.
