new job-sickness record

Hello
Ive just been offered a new job but one of the forms I have to fill out is sickness record. Sadly my sickness record has been awful over the last 2 years due to depression mainly but also migraines and an episode of d&v.
A lot of my depression is linked to work so new job is a completely new area and also closer to home etc meaning I can have a better work life balance etc. I have also now changed meds and stuff.
I am scared though that they will retract the offer due to my sickness record and I will forever be in a loop of sickness as i won't be able to leave my job.
Has anyone any experience of this? Sorry waffling on but I really need this change!

I would attach a confidential letter to explain the circumstances of your time off. I had an extended period off following a bereavement, so I explained the context of that period of time.

I would let them know you have MH issues as it means if later down the line you need to request reasonable adjustments then you are already on their radar re Equality Act. They would be unlikely to retract their Employment Offer purely because of your health declaration, however if you believe this job will improve your MH challenges in terms or reduced stress levels and work life balance, it wouldn't do any harm to mention your health circumstances are under control and you are looking fwd to joining them.

I wouldn't get any more specific, you have no obligation to tell them confidential information about your health, only sufficient so they know you are motivated towards good attendance.
