I am interested in any views or experience on my current dilemma.

I work ft in a reasonably demanding professional job. I earn £40000. I love my colleagues and the flexibility is great. The prospects of any promotion or change in responsibility are very low. I will essentially be doing the same job for the next 20 years. Usual crap management politics but very supportive line manager.

I have an interview for a similar job in the nhs , paying £8000 more, but essentially the same responsibility. The team is much smaller and it would be a risk as to whether they were supportive ...
I really need the flexibility I have atm, at best it would take time to build that up again.

Would you risk changing in my situation (presuming I get It!) ? Also what questions should I ask to clarify things?

An interview is about the fit so you should ask questions about flexibility of the role

Interviews are two-way - this is your opportunity to see if it will offer you what you need from a job.

What the others said. It’s a two-way process so go along and see what you can find out.
