Could anyone please help me write this letter?

I started a job two week ago, and, this week have had to be off sick.

Due to dd1 now being ill and the possibility of another job, I am going to stop working there.

Could anyone please help me word a nice letter please?

Agree with scootergirl - make it nice and pleasant but be honest with them that you feel sad to have to hand your notice in, but that you feel you they need someone who can provide more than you are able to give at the moment.
