Reducing hours at work

How do I go about reducing my hours at work? Im hoping to reduce from 37.5 hours to 30 hours a week due to my health. I know the chances of me getting reduced hours are slim as we are always short staffed, but going to give it a try and see what happens.
Do I need to write a letter or can I just speak to my manager?

Here's the process.

I would suggest ensuring you address the question of how you think your work can get done in fewer hours in your application and come up with solutions to the various concerns your employer might have. If you just ask to reduce your hours without addressing these things you are less likely to be successful.
