Think about how you would react in different / difficult situations
Are there any KPIs or similar to familiarise yourself with?
Think about how you build relationships with your patients in their own home?

Good luck x

for nursing in general make sure you know about the 6c's and the patient experience etc. for community i would be looking at the darcy report and the 2020 vision.
good luck.

Definitely the six C'c and the Five Year Forward plan.
Community nursing near me has had a massive change, the matrons and DN's work in integrated teams, maybe have a look at the structure of the organisation.
Lone worker policy as well.
What would you do if you went to a patients house and there was no answer?

diamondimelda · 05/10/2015 19:48

If no one answered I would check if I could see if they had fallen etc and you can enter to give emergency help. I no sign I would ring my band 6 and see what they advise. ( I think)

The 6 cs best of luck!

Hi OP
In my area there is lots of integration going on- "hubs" created to work with neighbourhood care teams that include MH services, long term conditions, palliative care and cover days and nights. Also integration of acute care and community care (in-reach and outreach nurses). Big focus on keeping pressure off a&e/ambulance services so lots of preventative and proactive care.
Good luck! Xx
