Advice on interview, disclose upcoming surgery?

I have a dilemma. Applied for a new job in a new company and need some advice pre-interview (if I get one, no reason to think not)

I'm on a waiting list for surgery that will see me off work for 3months (as advised by consultant). I was referred in Dec 2014 and the latest update is that I won't hear anything till around August this year.

What's the rule of thumb in disclosing this in an interview/job offer?? It doesn't sit well with me to not mention it as I feel it's only fair etc however I really want this job and feel it may go against me (not that I'd blame them)

I have no date for surgery. As far as I know it could be August, next year or next wk if a cancellation came through! Totally stuck!

Has anyone got a HR angle on this? TIA, also going to post this in chat.

There is absolutely no requirement for you to disclose your upcoming surgery at interview.
