Okay, I'll bite.

The form is only issued after a telephone conversation with a call handler.

The form is specific to the caller because, as has been linked , you have to give your name, address, national insurance number and approximate annual income before they issue the form, which has a unique serial number printed on it, so that it matches with the file they create for you.

The form takes around 2 weeks to arrive after you phone and they will backdate the claim by up to a month only.

maybe you can write them. i've found the address in liverpool. i believe they require your national insurance number. not sure what else. maybe someone else know better (what information is required before they'll send you a pack...)
