A few questions about Flexible Working...

First off, should I write a very formal letter or form (like this one [www.direct.gov.uk/prod_consum_dg/groups/dg.../dg_179564.rtf
here]) or is it OK to write a more personal and relaxed one? Better to have a chat too? There is a good ambience in my office and I think it would seem awkward if I just hand over a template.

And then, who should I give it to? My line manager, the head of our department or HR?
