Extending maternity leave and requesting reduced hours

Wondering if anyone can help me with regards to extending my maternity leave and requesting reduced hours.
I've currently been on leave for 6 months and after me and my partner discussed income, we've decided we can afford for me to take the full years maternity entitlement.
I'd also like to return to work on reduced hours (ideally going from 5 days a week to 3).
I know I have to request these changes in writing, but should I put both requests in the same letter? Should I ring and speak to my HR dep first so they know it's coming.

All advice welcome.

This site has loads of really helpful information. Especially the flexible working and work life balance bit.

Make sure you put everything in writing and keep copies for yourself. I can't tell you how many problems I'm having with this at the moment! I would also ask if they have a specific form for you to fill in to request flexible working hours, I was told to just write a letter and now 2 months later am being told I should have filled a form in (which I didn't even have access to!).

If you want to extend your leave I think you just give 8 weeks notice, but I should tell you on that site.

I would extend your mat leave first, then contact them about changing your hours nearer the time of your return to work.
