How do i ask my employer to change my hours after mat leave?

I am going back to work in afew months, i'm on maternity leave atm. I need to change the days i work otherwise i'll need to find another job. How do i go about this? Is there a procedure i need to follow? Any template letters i can use? Thanks.

The ACAS website has template letters to request flexible working and advise on doing so.
