Advice /etiquette for going back to old job

I had a part time job before I got pregnant, which I intended to return to. Unfortunately, I then got ill when pregnant and had to take sick leave then early maternity leave, somewhat leaving my employers in the lurch (but not really, I also helped them find mat cover ...)

I definitely don't want to go back, for various reasons, but I don't know how to phrase it

I haven't spoken to my boss since I was signed off sick. I have emailed him a few times but not got a reply. He is not a very nice man, and quite exploitative and paranoid (the main reason I'm leaving). When I first told him I was pregnant he said that 'we don't do maternity leave' and tried to convince me to come back to work after 3 wks

Technically I am not due back till may, but feel I should let him know ASAP that I've no intention of going back. But how? I would also quite like my accrued annual leave paid for, but I think if I ask him for that he will go crazy. It's a small industry so I don't want to get in his bad books, and just want to be polite and professional. Does anyone have any advice?
