Ok so I (finally) got my MATB1 form from my midwife today so I can give it in at work tomorrow, but what else do you have to tell them and how? Do I just write it on a piece of paper? I need to do it in advance before going in to work as it has to be sent off to head office and know there wont be anyone else at work who will be able to tell me what I have to give in.
