teacher's letter of resignation after maternity leave

Can anybody help me with a format/advice on how to write one? I've been staring at the screen for an hour now!

think i'd do two maybe - one very formal short one 'i wish to terminate my employment from *[date]' and a perfunctory 'thank you' as this one goes to the finance office and LEA, and then a longer letter to the head which could be more personal (that's if you're on good terms with him/her and it's all amicable of course!)

I just said that I had decided not to return to work for family reasons, thanked the head (and the staff) for their support. Said I had enjoyed working there and wished them well in the future.

Mind you, everyone knew by that point that I wasn't coming back, so it was just a formality.

The head replied with a letter thanking me for my work, wishing me well and telling when my contract officially ended.

Have you actually gone back to work and now you're resigning or are you just not planning to return?

Dear [whoever it's addressed to],

As required by my contract of employment, I hereby give you [however much] notice of my resignation from my position as [job title].

My [son/daughter] is now [x months] old and although I understand that I could return to work after my maternity leave, I have decided against this.

This was not an easy decision and took a lot of consideration. However, please be assured that I will do all I can to assist in the smooth transfer of my responsibilities.

I wish both you and everyone at [school name] every good fortune and I would like to take this opportunity to tell you how much I have enjoyed working with everyone there.

Thanks for all the help - used your format LeahE but wasn't sure of any date I should put (I started mat leave in July) so just left that out! I've emailed that one as they needed it urgently but will send a copy with a more informal letter to the head and chair of governors as suggested.
