Not going back to work but need help

Hello, I started my maternity leave in Jan 12 and had my beautiful baby boy in Feb 12. I am due to start back work in Oct 12 I have decided not to return back as I already have another son. I really need help with my letter, any help would be grateful. And also, in the letter, when I'm stating effective from DATE, what date is it, do I put in the date I'm due back or the date of the letter

Put all the dates in. ie

As you are aware I am due to return from maternity leave on date 1. I would like to give my notice from today date 2, with a 4 well notice period my understanding is that my contract will therefore end on date 3 (may be the same as date 1).
