Request for flexible working template letter

I am going back to work this September from ML. I would like to work 3 days a week with 1 day from home.

Does anyone know if there is a template/example letter on the internet I can take a look at?

I don't have a template letter, but the basic points to include are:

• The reason for the request (i.e. to care for your child under the age of 18)
• The working pattern you are requesting (days/hours - if you have any flexibility on this it would be helpful to include, to show willing)
• Proposed start date for the change
• What effect the flexible working pattern will have on your employer and how this impact can be mitigated
