Teachers - part time request letter help

I need to put in writing my request for part time hours. I also plan to still be breastfeeding my DD then, so plan to express at work.

1, Is there a standard request letter I can copy?
2, Do I have to inform them of continuing breastfeeding especially as I am a science teacher for health & safety?
3, Can I request a room/time for expressing even though DD will be 11 months?
4, Do I keep the breastfeeding bits for a separate letter?

TIA

There is a direct gov pro forma on their site i think under flexible working. Your local authority probably have their own too which you could ask for. My advice would be o get the application in asap as i am having real issues trying to change childcare when they rejected it. If you make them aware of the breastfeeding they have a duty to support you in this so i would notify them, not sure if in that letter though.

Confused I think I'm valued there! To be fair though I told my HoD what hours I'd want on my return prior to going on maternity leave, so she's planned staffing around it already....

I didn't think to check union website - brain seems to be running on slow for some reason - (oh, that might be the fact that I haven't had more than 2 hours sleep in a row for 8 months! Grin)
