Occupational health check for new nhs job

I have been offered a job in the nhs as a customer service officer on switchboard.
I am a little worried as I have had quite a few sick days in the last year and a half. Once I was signed off sick for 5 days for kidneys infection and I few other times a day or two here and there for tummy bug....flu/vomiting. What should I say on the questionnaire and can they ask my manager about sick days? He will give me very nice reference I know but what about the sick days? Can they refuse employment because of it?
Any advice appreciated thanks

But a friend of mine said that this form goes to human resources not to my future manager who was interviewing me....anyway if it comes to it I will explain it all.

Yes, I wouldn't worry about it, I'm surprised it even came up at interview.

It will be fine, your sickness record sounds like someone who has had kidney problems as well as the normal D&V sickness bugs type of days off.

Just tonight I have sat at home creating new transfer files and some of the sickness records look Shock, it's only when you start looking at the reasons behind the sickness that you can start to build a picture.

I gave my notice at work today couldn't wait anymore. Also remembered last night that I have used up all my holidays and I owe them 5 days back so was thinking if I wait it would take longer afterwards. So I gave 2 weeks notice plus a week on top to pay my holidays back! If it all goes wrong now I am in trouble Grin

The strangest thing happened yesterday. My manager said I don't need to work my three weeks notice but stay home and rest before the new job. She will pay my notice anywayGrin isn't that the best boss in the world? Made me feel bad that I am leaving! I don't have a start date yet for new job but am already home....feeling a bit scaredSadSad
