medical screening form

I know this has been done before and but.. I have acepted a job - very happy - interesting, not quite full time, and been out of work for a few months so very welcome .

I have got a medical form today and just not quite sure what to put.

I have had a couple of episodes of depression in the past - however the last one was more than 2 years ago - and was well controlled - I only had 2 weeks off - and the only one that was serious was more than 4 years ago - had about 4 months off
so one question is do you have any health problems which may affect your ability to do your job -
am I alright to say no to this one as I am perfectly well at the moment?

the other question: is have you ever had any illness which may have been caused or made worse by your work?

the serious episode wasn't helped by my work situation at the time- although it wasn't the main reason - I am just worried if I don't disclose it and then I become ill again it wouldn't be good - however both me and my employer are keen for me to start as soon as possible so really don't want them to have to get medical reports etc if can be avoided

If I put something like do you think it would be ok
I had an episoide of depression over 4 years ago which although not caused by work, some aspects of my work siutation at the time may have an impact. However I recovered well and was able to return to the same role and a good performance level.

Tricky - if they found out that you had kept it 'secret' that could be gross misconduct - if you do write any thing, I would stress the fact that this was all a long time ago and unlikely to affect you in the future etc (so start the sentance with, 4 years ago - rather than hiding it mid sentance).

Just to say wrote something starting 4 years ago and start date confirmed yesterday so all well
