Help with Maternity Allowance claim form - self-employed

  1. Form asks What dates were you registered as self-employed?

I've filled in the date I registered, but what about the end date??

  1. I haven't started maternity leave yet, but there's nowhere on the form to say when I want to start?

The helpline is closed today, and I'd really like to get this finished and in the post! Grin

I'm too impatient, I've looked further on Directgov site and they send you a form out.

Nothing to see here.
