Applying for job after long term sick leave

I used to work in a nursery and left in January last year as I had to have treatment for breast cancer. I haven't returned to paid work (although I do volunteer work) as yet but there's a post that I've seen that would be great. I've got the form etc but there is the following question on it:

How much sick leave have you had in the past three years? Please give details:

Now, technically I left the nursery to have treatment and so I didn't take sick leave. However, this potential employer already knows me and knows that I have been ill.

How best to answer the question?

Honestly.
That way they can't speculate about why you were off. If you feel you are able to do the job as specified in the JD then just let them know in the details bit why it isn't a problem. I think these sections of the form are more to guage whether someone has a long term illness that will affect their ability to work, or, if they are likely to be someone who can't cope with work or is a lazy sod!
