CV advice: institution names

I have studied or worked in three places that have changed their names in the last two decades. Should I give the name as it appears on the certificates or the current names? That's for the educatinal places. For the workplace, do I give the current name, so that a reference on headed paper makes more sense?

Hi Merryberry,

As a recruiter I wouldn't normally be too bothered, but that's because there isn't normally a direct link between what my candidates have done before and doing now. Possibly depends on how relevant the places will be to the job you're applying for.

Does it look odd to use both? eg:

Current name (previously old name)
