Lost MAT B1 form...

Can I get a replacement? I've turned the house upsidedown looking for the bloody thing!

Any advice greatly appreciated!

Speak to your midwife, I am sure she will give you a replacement no bother.

I am sure you are not the first to have lost it - especially with pregnancy brain! Smile

They are allowed to issue a duplicate of one already issued, and as DancingHippo says, it must happen all the time!
