This email seems to have been wildly, and widely, misinterpreted.
IMHO what it represents is Robert Trivers explaining the "trans phenomenon" to Epstein. My understanding of the final sentence is that "we" refers not to himself and other "gender surgeons" (because he is not one) but to society or perhaps more specifically to what is happening in America:
"BTW we are now pushing the intervention earlier--so you notice your 3-year old son has trans tendencies, so now you intervene with hormones I would be frightened to do that but who knows?"

Trivers' "Biosocial Research Foundation" seems to exist only as a funded university research project. It received funding from Jeffrey Epstein amongst others.

This lecture, based on a ground-breaking study by one of the greatest evolutionary theorists since Charles Darwin, Robert Trivers, examines the curious case of honor killings, which seem to make no evolutionary sense—why would a father kill his own daughter and thereby eliminate half of his own genes from propagating into the next generation? The answer is to be found in who, exactly, is committing these murders and why.
In short, the vast majority of honor killings are conducted by fathers and uncles who murder young women who have been arranged to marry a first cousin but who have fallen in love with someone outside of the family.
