So, a surgeon known for amputations turns out to have an amputation fetish? If I had been one of his patients I would feel very violated, and would have some big questions about whether all measures were taken to avoid amputation.

I wouldn't want to be treated by a man with such obvious MH problems, you need real trust in your Dr and knowing he was mentally ill would take that trust away.

There's a chapter in Oliver Sacks book The Man Who Mistook His Wife For a Hat about a man who wakes up every morning with a strange leg in bed with him. He throws the leg out of bed and the rest of him follows, because it's his leg. He doesn't recognise it at his, and I think he wanted it amputated, I can't remember how it went, how he was treated, whether he was reconciled with his leg etc, (and I can't find the book, naturally), but it demonstrates there's nothing new about people rejecting their bodies.
As for the fetish, is there nothing that men can't turn into a bluddy fetish? This one reminded me of Crash by J G Ballard, of which I have but the most basic of knowledge because I have the nous to not engage with things that will haunt me forever. But the most basic is quite enough.

Sacks actually later experienced the phenomenon himself after breaking his leg very badly. In his case it resolved rather than being a permanent condition. The brain's map.of the body is a fascinating thing (see also phantom limbs as the other side of the coin).

But neither of those cases were fetish related. Or involved the self-absorption of making it into their persona and chasing media attention for it.
