Charles Inglefield surgeon at London Transgender Clinic found fitness to practise impaired

A Medical Practitioners Tribunal found cosmetic surgeon Dr Christopher Inglefield misled a serious incident review meeting.

His actions were found to be dishonest.

'The tribunal found that, at a serious incident review meeting on 16 November 2018 with the Phoenix Hospital Group, Dr Inglefield stated that his practice (London Bridge Plastic Surgery and Aesthetic Clinic) had informed the Care Quality Commission (CQC) of a serious clinical incident, which occurred on 7 August 2018. The tribunal found that Dr Inglefield knew that, at the time of the meeting, the CQC had not been informed and that his actions were dishonest.

The tribunal has found Dr Inglefield’s fitness to practise to be impaired by reason of his misconduct. The tribunal will reconvene to consider what sanction, if any, to impose on Dr Inglefield’s registration.'

'In 2016, Dr Inglefield was given a 5 year written warning for repeated failure to properly complete surgical consent forms. He failed to record details & made revisions forms that weren't countersigned by the patient, who had a series of cosmetic breast procedures.'

I see that at least part of this is that he didn't get consent forms and can't help noting that was also one of the reasons that the Tavistock was rated as 'inadequate' by the CQC (www.bbc.co.uk/news/health-55723250)
Out of the ten records they checked where blockers had been referred, they'd found that only three in ten had completed consent forms.
You'd think filling out a consent form would be fundamental to proceed.
And if the forms aren't being done - even as a tickbox exercise -, how much confidence do we have that girls are being given the information they need to make an informed decision?
They're being failed at every stage of this pipeline.

It's not quite clear what the 'major incident' of 2018 is or what happened subsequently, as far as I can tell. Hoping Tribunal Tweets are given more information.

Ahh. I knew I'd seen something about him being sanctioned regarding consent forms but when I looked I couldn't find it. I wondered if he'd managed a google clean up. Well, good to see justice catching up with him.

Personally I think getting your kit off to pose with young patients like that is worthy of a finding of impaired fitness to practice.
I hope they look at more than the one major incident whatever that was.

Page 12 has this broad definition: "1. What is a Serious Incident?
In broad terms, serious incidents are events in health care where the potential for learning is so great, or the consequences to patients, families and carers, staff or organisations are so significant, that they warrant using additional resources to mount a comprehensive response. Serious incidents can extend beyond incidents which affect patients directly and include incidents which may indirectly impact patient safety or an organisation’s ability to deliver ongoing healthcare.

The occurrence of a serious incident demonstrates weaknesses in a system or process that need to be addressed to prevent future incidents leading to avoidable death or serious harm7 to patients or staff, future incidents of abuse to patients or staff, or future significant reputational damage to the organisations involved. Serious incidents therefore require investigation in order to identify the factors that contributed towards the incident occurring and the fundamental issues (or root causes) that underpinned these. Serious incidents can be isolated, single events or multiple linked or unlinked events signalling systemic failures within a commissioning or health system."

Just realised I misnamed the surgeon in the OP! It is of course Christopher Inglefield.

He apparently made a video for Instagram apologising to clients for failures in October last year.

This has since been deleted, but there's a fairly detailed description of it on Reddit:

'Regrets the decision to recruit inexperienced nurses in terms of being able to show the empathy patients deserve with “substandard” post and pre operative care.'

had a couple of situations with GCS patients where the surgery was fine, but the post op care “was not ideal” and has led to the LTC and Harley Street Hospital to suspend GCS until a full independent investigation into the process of the care pre, during and post operatively for patients

I wonder if this is the "serious incident".

NHS England is funding a new surgical team for "masculinising genital surgery" at Chelsea and Westminster hospital with further provision at a couple of private hospitals. The plans include recruitment of Two whole time Tissue Viability Nurses. It's almost as if they expect post operative complications.

The NHS should not be funding any surgery which requires TVNs unless it is physically lifesaving.

I believe that gender dysphoria is a mental health issue and should be treated as such. If it isn't a mental health issue, it doesn't require any treatment. There is no evidence at all that surgery alleviates any distress.

It's almost as if they expect post operative complications.

Given the stats, it's pretty much guaranteed:

(Bearing in mind that is a site that is promoting surgery, I presume it's showing the surgery in best possible light. Yet it still reports staggeringly high levels of really serious complications - fistula, infections, incontinence, loss of member, strictures, etc)

It's appalling OP. I've just noticed from the FOI it is one whole time TVN, not two as I said.
Still extraordinary. My hospital has two or three, I'm not sure if they are all full time, for a 600 + bed hospital.
This will be for about 250 patients a year.
