ooh brilliant op - I was just coming over to see if this had been posted.

It is quite good - it asks for Sex upfront although I did have a sharp answer to the 'gender identity' question.

Lots of good free text boxes with open questions to lay the issues out to them clearly. I wish I knew how they read them so as to format my answers in the most effective way but I have given them some food for thought - mainly around the need for single sex facilities and to stop erasing the word 'woman' from health literature and services.

Will be completing on behalf of my sister (and mum). Both are disabled and we are all sick of the poor accessibility of health services that regularly impact their comfort, dignity and safety.
