I'm more worried about names and email addresses being captured from those filling out the survey (am I being paranoid?).

I wasn't keen on that either. You could use a fake name and a "disposable" email address that will allow you to spot any subsequent emails using that address:

Search for "disposable email address". Some expire after a while and some don't.

I have got some spare Gmail addresses that I use and add something like this before the @ sign so I know if an email address used for registering somewhere has been passed on - more info here:

I’m the ‘GDPR gnu’ in our company - so I’ve done all the training and everything... I’m also a pita when it comes to organisations breaking the rules.
