Examples of sex and gender being confused or conflated

Forgive me if this has been done before, but I thought it might be handy to compile a list of official bodies that ought to know better than to use "sex" and "gender" incorrectly. To wit:

Metropolitan Police, on a crime report form:
Do you feel race, ethnicity, sex, gender, sexuality, disability or religion were a factor in this crime?

The NHS (verbally at least).

Last time I gave blood, the form you fill in beforehand asked for your gender rather than sex. I nearly crossed it out, and I definitely will next time.
