This is a standard police procedure and occurs following:

  • incidents where someone is hurt as a result of direct or indirect police action
  • the arrest of a police officer/member of police staff
  • all domestic homicide reviews/serious case reviews that involve police action/inaction
  • all dog bites, taser discharges and firearms incidents
  • any police pursuit that results in a collision

Voluntary referrals also take place in any other circumstances - usually when it is likely that there will be questions about the police's handling of a situation or an individual (eg "a near miss" in the custody suite when dealing with a detainee with severe mental health problems, for instance)
